About The Position
The Intensive Case Manager provides services to outpatient psychiatric clients, identifying and supporting vulnerable community members. Case management includes providing short-term intensive care coordination with partner agencies, housing resources, and healthcare providers, as well as continued mental health stabilization and assisting clients' success in their community for a given period. They will continue to work with some clients for an extended period after transfer to adult case management services. The Group Therapist coordinates and conducts group psychotherapy, including clinical assessment of patients, brief psychotherapy, crisis intervention, referral, care coordination, and transition. Requirements
- Master's Degree in Mental Health counseling or equivalent.
- 2 years acute care or community-based mental health work.
