This position is within the Behavioral Health Services department, specifically on the Assertive Community Treatment (ACT) Team.

Requirements

  • Current resident of the state of Wisconsin at the time of application.
  • Valid Wisconsin driver’s license, able to be maintained throughout employment.
  • Master’s Degree or higher in Human Services, Psychology, Social Work, Occupational Therapy, or Nursing (RN).
  • At least one (1) year of experience providing services to individuals with Serious and Persistent Mental Illness (SPMI) in the community.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Available and willing to be part of a rotation for after hours and weekend on-call services.
  • Able to lift/carry and push/pull up to 30 lbs.
  • Willing to travel within the community ~75% of work time.

Nice To Haves

  • Valid Wisconsin licensure without restriction as a LPC, LCSW, LMFT, APSW, or LPC-IT.

Responsibilities

  • Provides direct case management services to help the consumer successfully live in the community.
  • Provide brokerage of services outside of ACT team when needed, regular collateral contacts of providers, and others involved in the consumers' lives.
  • Conduct individual assessments, including in-depth assessment at time of intake. Complete routine assessments thereafter at least every six months.
  • Works with consumer and natural support system to establish rehabilitative goals to further the consumer's recovery process.
  • Work in close collaboration with the consumer in their home to engage in psychiatric rehabilitative services to develop and enhance the consumer's independent living skills. This includes side by side training of ADL skills such as housekeeping, laundry, grocery shopping, and use of public transportation.
  • Provide crisis response services to individuals who are experiencing a psychiatric crisis.
  • Interface with county operated crisis services, hospitals, and others as needed.
  • Includes being part of a rotation of after hours on call services provided by the ACT team.
  • Use Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and other evidenced based therapeutic practices, provide individual and group therapy as well as psychoeducation groups to ACT team consumers.
  • Complete clinically sound and timely documentation of all services rendered on behalf of consumers served by ACT team in the Electronic Medical Record.
  • Maintain a productivity level of 75% or greater.
  • Provide services in various weather and household conditions.
  • Perform all other duties as required.
