The Mental Health Clinician, Safe Options Support (SOS) position involves community outreach on the streets and subways, coordinating participants' needs before and after their move from street to home, enhancing their daily living skills, providing supportive counseling, and advocating on their behalf when faced with discrimination or healthcare inequities. The role requires persistent and assertive engagement using strength-based approaches, partnering with various outreach teams and agencies to identify those in need, and continuously assessing participants' health and social needs. This role also collaborates with the SOR Hub for housing, participates in hospital discharge planning, provides short-term therapeutic counseling, supervises case managers and peers, collects and reports data, resolves clinical issues for housed participants, fosters relationships with community providers, navigates appointments, and facilitates crisis interventions. Additionally, the clinician will review documentation, conduct comprehensive psychosocial assessments, obtain historical information, and monitor participant progress.
