The Homeless Outreach Stabilization Team (HOST) is a multi-disciplinary team that works in the community to provide services including substance abuse treatment, medical services, rehabilitative support, case management, and peer support services. The program's purpose is to serve homeless individuals with behavioral health challenges who have demonstrated continuous service needs and functional impairments and have not benefited from traditional outpatient programs. The Mental Health Clinician II will collaborate with a multi-disciplinary team serving homeless adults with severe and persistent mental illness and substance use disorders. This role provides targeted services to individuals who are highly vulnerable, lack service connections or other support, and are unable to engage in traditional service models. The clinician will collaborate with the medical team, provide relevant psychoeducation to the larger team, and conduct clinical case review and staffing with other team members. Services will be provided in various community locations such as emergency homeless shelters, transitional homeless shelters, homeless encampments, transportation hubs, train stations, and other pertinent community locations where people may gather.
