Child and Family Focus is hiring a Mental Health Case Manager in Chester County! The First Episode Psychosis (FEP) program uses a Coordinated Specialty Care approach to support young people experiencing their first episode of psychosis. Services include medication management, therapy, family education, case management, supported education and employment, and peer support. The program focuses on empowering individuals to guide their own recovery, build relationships, and successfully reintegrate into school, work, and the community! The Case Manager develops and coordinates services based on individual service plans, providing case management, service coordination, supportive counseling, and advocacy to help clients achieve recovery goals and build self-advocacy. This role works within a multidisciplinary team to deliver community-based support to a caseload of up to 30 adolescents and young adults (ages 15–30) experiencing their first episode of psychosis.
