Requirements

  • Equal to completion of 4 years of high school plus the completion of additional courses/training in community health work or behavioral health is required.
  • Demonstrated ability to perform as part of a multidisciplinary team.
  • Must have basic competency with documentation and computer skills.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Nice To Haves

  • Bachelors degree preferred.
  • Culturally competent with solid knowledge of the communities being served.
  • Community Health Worker and/or Recovery Coach certification preferred.
  • Experience with EMR in medical settings.
  • Bilingual skills.

Responsibilities

  • Work as a member of the behavioral health team to expand organizational support for equitable and accessible health management of behavioral health (BH), substance use disorders, and social determinants of health (SDOH).
  • Provide culturally appropriate health education and utilize critical thinking to navigate systems.
  • Facilitate the coordination of care and community resources to meet the patient's plan of care.
  • Work in a team-based role and participate in team activities.
  • Promote patient autonomy, address barriers to support treatment recommendations and optimal disposition planning for patients with a range of behavioral health, social, and medical concerns.
