About The Position

Performs basic, routine duties related directly and indirectly to clinical care of military veteran patients and their family members. This position will cover Atlantic Health Newton Medical Center & Atlantic Health Hackettstown Medical Center.

Requirements

  • Must be able to perform basic, routine duties related directly and indirectly to clinical care of military veteran patients and their family members.
  • Must be able to run daily admission reports.
  • Must be able to review charts and communicate with clinical team, patients, and family members.
  • Must be able to assist with linkages to community resources.
  • Must be able to meet and greet identified military veteran patients at bedside.
  • Must be able to assist clinicians with care coordination tasks, including calling, emailing, and faxing information with a signed ROI.
  • Must be able to discuss patient treatment plans, symptoms, and other pertinent information with care teams and outside providers with a signed ROI.
  • Must be able to assist clinicians with sending and managing disability and human resources forms.
  • Must be able to appropriately document patient contacts, phone calls, group notes, and other patient information in EMR.
  • Must be able to assist with inputting evidenced-based screenings and other intake documents in EMR.
  • Must be able to participate in multi-disciplinary treatment team and general business meetings.
  • Must be able to orient patients to the program and develop rapport with patients, family, and significant others.
  • Must be able to apply principles of infection control and safety.
  • Must be able to assist in keeping patient care and non-patient care areas clean.
  • Must be able to safely and effectively utilize the hospital computer system.
  • Must be able to attend community and stakeholder meetings related to military veterans.
  • Must be able to participate in orientation of mental health assistants.

Responsibilities

  • Runs daily admission report to check if new patients who are military veterans have been admitted into the hospital.
  • Reviews charts, communicates with clinical team, patients and family members to determine need for resources. Assist with linkages to resources in the community.
  • Meets and greets identified military veteran patient admitted into the medical center at bedside if appropriate.
  • Assist with care coordination tasks such as calling, emailing and/or faxing information to PCP or other providers, with a signed ROI from patient.
  • Discusses patient’s treatment plan, symptoms, and other pertinent information with patient’s care team including outside providers, with a signed ROI from patients.
  • Assists with sending (email or fax) and managing disability and human resources forms and/or information for patients who are in treatment.
  • Appropriately documents patient contacts, phone calls, group notes, and other patient information in EMR as necessary.
  • Assists with inputting evidenced-based screenings and other intake documents in EMR as assigned.
  • Participates in multi-disciplinary treatment team and general business meetings.
  • Orients patient to program and develops rapport with patients, family and significant others as needed.
  • Applies principles of infection control and safety in the work environment.
  • Assists in keeping patient care and non-patient care areas clean (e.g., wipes tables, neatens storage rooms, pantries and other areas).
  • Safely and effectively utilizes the hospital computer system in accordance with hospital policy.
  • Attends community and stakeholder meetings related to military veterans as a representative for True North.
  • Participates in orientation of mental health assistants.
  • Completes other responsibilities as assigned.
