Within the context of a client and community care model and in accordance with the standards of professional practice and the established vision and values of the organization, works as a member of an interdisciplinary team to conduct the centralized intake functions for Mental Health & Addictions Services to ensure linkages between client and the appropriate resources occurs. Conducts the intake function for individuals with mental health and/or substance abuse issues and performs initial screening, triage and referrals based on client needs, urgency of request and best practices to determine the appropriate treatment. Develops, implements and adjusts outcome orientated individualized treatment plans that covers both the mental health and substance use disorder. Provides treatment/support services to clients including individual and group therapy, couples and family therapy, and education, manages the waitlist and provides consultation to client/family. Provides information to health providers and the general public, establishes and enhances relationships/communication within Mental Health & Addictions Services and other community resources, prepares and maintains client records, maintains and provides accurate intake/referral data and statistics, participates in team meetings and continuing education, and engages in on-going team planning and quality improvement.
