About The Position

The Memory Care Support Specialist manages and implements the programs, philosophies and standards for the Memory Care Center.

Requirements

  • CLINICAL AND HUMAN SERVICES KNOWLEDGE: UNDERSTANDING SOCIAL WORK PRINCIPLES, PSYCHOSOCIAL ASSESSMENTS AND INTERVENTIONS. STRONG UNDERSTANDING OF PERSON-CENTERED APPROACH TO RESIDENT CARE. EXPERIENCE AND KNOWLEDGE OF DEMENTIA RELATED BEHAVIORS, STRATEGIES AND BEST PRACTICES.
  • BACHELOR’S DEGREE IN SOCIAL WORK OR IN A HUMAN SERVICES FIELD REQUIRED.
  • STRONG PASSION FOR GERIATRIC ADVOCACY AND COMMITMENT TO SENIOR CARE EXCELLENCE.
  • DEMONSTRATES C.A.R.E. VALUES TO OUR RESIDENTS, FAMILY MEMBERS, CUSTOMERS AND STAFF. COMPASSION, ACCOUNTABILITY, RELATIONSHIPS AND EXCELLENCE

Nice To Haves

  • Long-term care experience preferred
  • PREVIOUS SOCIAL SERVICES EXPERIENCE PREFERRED.

Responsibilities

  • Manages and implements the programs, philosophies and standards for the Memory Care Center.
  • Train, observe and redirect staff through initial education and one-on-one education of the Alzheimer disease process.
  • Empathetic engagement with residents and families.
  • Clear, effective communication with interdisciplinary teams.
  • Conflict resolution and mediation skills.
  • Creating a comforting and engaging atmosphere for our residents.
  • Works with and promotes collaboration among departments, professionals, consultants, and organizations including government regulatory agencies.
  • The ability to work towards a common goal of excellent care for our residents.
