About The Position

The Memory Care Support Partner role is a PRN position at The Fremont, located in Springfield, Missouri. This role focuses on providing empathetic and patient-centered care to residents in memory care neighborhoods. The position involves creating human connections, understanding resident interests and abilities, preparing the environment for daily activities, partnering with residents throughout their day, documenting care, and assisting with team member orientation. The starting wage for this position ranges from $18.00 to $22.75 per hour.

Requirements

Must be at least eighteen (18) years of age.
High school diploma, or equivalent.
Ability to read, write, understand, and communicate in English with residents.
Positive and energetic attitude.
Computer skills to be able to print documents and enter information about residents.
Must be active as this role requires standing, walking, bending, kneeling, and stooping all day.
Must have the ability to frequently lift and/or move items up to 50 pounds.
Must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.
Must be criminally cleared.

Responsibilities

Be responsible for understanding the importance of empathy and creating human connection with residents during their most vulnerable moments.
Discover residents’ interests, abilities, and what makes each resident who they are.
Collaborate to create appropriate engagement plans and implement strategies with patience to ensure success.
Be responsible for setting the stage for a successful day, including preparing dining rooms, setting up spaces for events, and preparing resident rooms for nighttime routines.
Partner with residents to achieve all facets of daily life from the moment they wake to the moment they go to sleep.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
