About The Position

Our Memory Care Support Partners are responsible for creating human connection with residents during their most vulnerable moments, understanding their interests and abilities to create appropriate engagement plans, and setting the stage for a successful day by preparing environments like dining rooms, libraries, and resident rooms. They partner with residents throughout their daily lives, from waking to sleeping, and are also responsible for documenting resident care and changes in condition, as well as assisting with the orientation of new team members in memory care neighborhoods.

Requirements

Must be at least eighteen (18) years of age.
High school diploma, or equivalent.
Able to read, write, understand, and communicate in English with residents.
Positive and energetic attitude.
Possess computer skills to be able to print documents and enter information about residents.
Must be active as this role requires standing, walking, bending, kneeling, and stooping all day.
Must have the ability to frequently lift and/or move items up to 50 pounds.
Must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.
Must be criminally cleared.

Responsibilities

Be responsible for understanding the importance of empathy and creating human connection with residents during their most vulnerable moments.
Discover residents’ interests, abilities, and what makes each resident who they are. Collaborate to create appropriate engagement plans and implement strategies with patience to ensure success.
Be responsible for setting the stage for a successful day, from preparing the dining room for breakfast to setting up the library for an event, to preparing the resident’s room for their nighttime routine.
Partnering with residents to achieve all facets of daily life from the moment they wake to the moment they go to sleep.
Be excellent at documenting resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
