About The Position

Our Memory Care Support Partners are responsible for creating human connection with residents during their most vulnerable moments, understanding their interests and abilities to create appropriate engagement plans. They are also responsible for setting the stage for a successful day by preparing the environment for various activities and routines, and partnering with residents throughout their daily lives. This role requires excellent documentation of resident care and changes in condition, as well as assisting with the orientation of new team members in memory care neighborhoods.

Requirements

  • Must be at least eighteen (18) years of age.
  • High school diploma, or equivalent.
  • Ability to read, write, understand, and communicate in English with residents.
  • Positive and energetic attitude.
  • Computer skills to print documents and enter information about residents.
  • Must be active as this role requires standing, walking, bending, kneeling, and stooping all day.
  • Ability to frequently lift and/or move items up to 50 pounds.
  • Must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.
  • Must be criminally cleared.

Responsibilities

  • Create human connection with residents during their most vulnerable moments.
  • Discover residents’ interests, abilities, and what makes each resident who they are.
  • Collaborate to create appropriate engagement plans and implement strategies with patience to ensure success.
  • Be responsible for setting the stage for a successful day, from preparing dining rooms to setting up for events and preparing resident rooms for nighttime routines.
  • Partner with residents to achieve all facets of daily life from the moment they wake to the moment they go to sleep.
  • Be excellent at documenting resident care and changes in condition.
  • Assist with orientation of team members assigned to memory care neighborhoods.
