Requirements

  • High School Diploma or General Education Degree (GED).
  • One year of direct care experience working with the Alzheimer’s population preferred.
  • Experience providing personal care and assistance with ADL’s required.
  • Minimum 18 years of age.
  • A valid QMAP certification is required for the Memory Care Specialist QMAP position.
  • Colorado Identification Card.
  • Food Handler’s Card (required within six months of employment).
  • Hilltop Dementia Certification (required within six months of employment).
  • Qualified Medication Administration Personnel (QMAP) Certification.

Nice To Haves

  • CPR Certificate/First Aid Certificate.

Responsibilities

  • Demonstrate ethical and professional behavior aligned with Hilltop’s Assisted Living Communities (ALCs) values, including Service 1 (how we treat each other) and Service 2 (how we treat our residents).
  • Understand and strictly follow all applicable Hilltop, Fountains, and state and federal rules, regulations, policies, and procedures.
  • Maintain professional boundaries with all residents, while upholding their dignity, confidentiality, services, and rights.
  • Provide protective oversight, including assisting residents, maintaining a safe physical environment, redirecting and assisting disoriented or confused residents, applying and following house rules, honoring resident rights, appropriately reporting and assisting residents experiencing difficulties, and identifying and monitoring visitors.
  • Provide social supervision, including helping with transportation and ambulation to events, assisting with communication and attendance at social events, and assisting with general socialization.
  • Provide for the sanitation and safety of residents, including monitoring the safety of living and common areas, removing and reporting safety hazards, maintaining safety supplies, redirecting residents from unsafe areas or situations.
  • Provide personal care to residents as directed by the specific care plan and appropriately document care given.
  • Monitor ambulation and assist residents to and from meals and/or activities.
  • Perform housekeeping duties including mopping, dusting, trash removal, linen changes, stocking supplies, vacuuming, disinfecting/sanitizing, cleaning bathrooms, bedrooms, living rooms, wheelchairs, and walkers, organizing apartments, and laundry.
  • Answer radio, pager, and pendant requests for assistance.
  • Assist residents with memory care activities and social supervision.
  • Order meals for residents and serve as needed, clean and sanitize the dining area.
  • Redirection and de-escalation of behavioral residents.
