Requirements

  • High School Diploma or General Education Degree (GED).
  • One year of direct care experience working with the Alzheimer’s population (preferred).
  • Experience providing personal care and assistance with ADL’s (required).
  • Minimum 18 years of age.
  • A valid QMAP certification is required for the Memory Care Specialist QMAP position.
  • Colorado Identification Card.
  • Food Handler’s Card (required within six months of employment).
  • Hilltop Dementia Certification (required within six months of employment).
  • Qualified Medication Administration Personnel (QMAP) Certification.

Nice To Haves

  • CPR Certificate/First Aid Certificate (preferred).
  • De-Escalation Training (preferred).

Responsibilities

  • Demonstrate ethical and professional behavior aligning with Hilltop’s Assisted Living Communities (ALCs) values (Service 1 and Service 2).
  • Understand and follow all applicable Hilltop, Fountains, and state and federal rules, regulations, policies, and procedures.
  • Maintain professional boundaries with residents while upholding their dignity, confidentiality, services, and rights.
  • Provide protective oversight, including assisting residents, maintaining a safe environment, redirecting confused residents, applying house rules, honoring resident rights, reporting difficulties, and monitoring visitors.
  • Provide social supervision, including assisting with transportation and ambulation to events, communication of and attendance to social events, and general socialization.
  • Ensure the sanitation and safety of residents by monitoring living and common areas, removing/reporting hazards, maintaining safety supplies, and redirecting residents from unsafe situations.
  • Provide personal care to residents as per their care plan and document care given.
  • Monitor ambulation and assist residents to and from meals and activities.
  • Perform housekeeping duties including mopping, dusting, trash removal, linen changes, stocking supplies, vacuuming, disinfecting, cleaning various areas (bathrooms, bedrooms, living rooms), cleaning mobility aids (wheelchairs, walkers), organizing apartments, and laundry.
  • Respond to radio, pager, and pendant requests for assistance.
  • Assist residents with memory care activities and social supervision.
  • Order meals for residents, serve as needed, and clean/sanitize the dining area.
  • Perform redirection and de-escalation of behavioral residents.
