About The Position

The Bronx Adult Day Care Center located in Water's Place Bronx is seeking a full-time Memory Care Program Coordinator to join our team! The Memory Care Program Coordinator provides direct supervision and oversight of staff to ensure the effective delivery of Memory Care services and programming. This position coordinates case management, care coordination, entitlements, and supportive services for Memory Care clients while ensuring all social work documentation complies with Department of Health (DOH) requirements. The Coordinator provides social work support to clients and caregivers, manages referrals, intake, and attendance for the GUIDE/Respite Program, and ensures timely completion of all required screenings, coordination activities, and documentation. The position also maintains ongoing communication with the Administrative Manager regarding all aspects of daily program operations.

Requirements

  • Providing or coordinating orientation and training for new staff, recreation interns, and volunteers

  • Coordinating clinical staff coverage and assignments
  • Providing in-service training for staff serving Memory Care clients
  • Evaluating workloads and motivating staff
  • Maintaining professional competence through continuing education, seminars, and training opportunities
  • Collaboration with Transportation staff
  • Reviewing transportation schedules
  • Ensuring staff assignments are developed and reviewed regularly
  • Ensuring monthly activity schedules are prepared, posted, and distributed
  • Reviewing activity schedules daily
  • Reporting gaps in program delivery
  • Coordinating, facilitating, leading, and participating in care planning meetings
  • Ensuring care plans are person-centered, individualized, and support continuity of care
  • Scheduling care planning meetings and preparing summaries
  • Distributing care plan summaries
  • Responding promptly and appropriately to participant emergencies
  • Collaborating with interdisciplinary team members during emergency situations
  • Conducting comprehensive reviews of participant health records upon discharge
  • Ensuring all required documentation is completed for discharge planning
  • Maintaining regular communication with the Administrative Manager
  • Serving as a team leader and collaborating effectively with interdisciplinary staff
  • Establishing and maintaining professional relationships with community agencies and social service organizations
  • Representing the Memory Care Program in community and professional settings
  • Performing other duties related to ADHCP operations as assigned

Responsibilities

  • Coordinate all aspects of the Memory Care Program, including intake evaluations and admission of new clients, attendance monitoring and follow-up, problem resolution, activity evaluation, staff oversight and supervision, and staff scheduling.
  • Complete initial and ongoing Social Work and Memory Care screenings and assessments for all clients enrolled in the Memory Care and Respite Programs.
  • Ensure client attendance is maintained according to authorized schedules and service plans.
  • Collaborate with Community Case Managers, respite referral agencies, and community mental health providers to address barriers to program participation.
  • Manage referrals, intake processes, and attendance tracking for GUIDE/Respite Program participants.
  • Ensure staff and aides carry out assigned responsibilities effectively and in a timely manner.
  • Report staff performance concerns to the Clinical Supervisor and Administrative Manager.
  • Monitor completion of daily documentation related to staff duties and client services.
  • Provide or coordinate orientation and training for new staff, recreation interns, and volunteers.
  • Review job responsibilities and program services with staff or designate appropriate personnel to do so.
  • Coordinate clinical staff coverage and assignments in collaboration with the Clinical Supervisor.
  • Provide in-service training for staff serving Memory Care clients, as needed.
  • Demonstrate leadership by evaluating workloads and motivating staff to perform efficiently and effectively.
  • Maintain professional competence through participation in continuing education, seminars, and training opportunities.
  • Collaborate with Transportation staff to ensure safe, efficient, and reliable transportation services for clients.
  • Review transportation schedules to optimize pick-up and arrival times.
  • Ensure staff assignments are developed and reviewed regularly.
  • Ensure monthly activity schedules are prepared, posted, and distributed to clients.
  • Review activity schedules daily to verify adequate staffing and program resources.
  • Report any gaps in program delivery to the Clinical Supervisor and Administrative Manager.
  • Coordinate, facilitate, lead, and participate in care planning meetings for Memory Care clients.
  • Ensure care plans are person-centered, individualized, and support continuity of care with community healthcare providers.
  • Schedule care planning meetings in a timely manner and prepare comprehensive meeting summaries.
  • Distribute care plan summaries to Community Case Managers and healthcare providers, as appropriate.
  • Respond promptly and appropriately to participant emergencies.
  • Work collaboratively with interdisciplinary team members during emergency situations, including the ADHCP Nurse and other relevant staff.
  • Conduct comprehensive reviews of participant health records upon discharge.
  • Ensure all required documentation is completed to support safe and appropriate discharge planning.
  • Maintain regular communication with the Administrative Manager regarding census, participant engagement, transportation concerns, staffing issues, and emergencies.
  • Serve as a team leader and collaborate effectively with interdisciplinary staff.
  • Establish and maintain professional relationships with community agencies and social service organizations.
  • Represent the Memory Care Program in community and professional settings, as needed.
  • Perform other duties related to ADHCP operations as assigned.
