About The Position

We are seeking a compassionate, experienced Memory Care Nurse (Wellness Manager - LVN/LPN) to join our team and help oversee the quality of care provided to our Memory Care residents. This role is ideal for a nurse who is passionate about dementia care, enjoys mentoring others, and thrives in a collaborative leadership environment. This position reports directly to the Wellness Director and plays a key role in supporting clinical operations, resident care, and team development within our Memory Care neighborhood. The Wellness Manager Position is: OVERNIGHT 10PM- 6AM Sunday to Thursday.

Requirements

  • Active LVN/LPN license in good standing
  • Experience in Memory Care, dementia care, or assisted living preferred
  • Leadership mindset with the ability to coach and support care team members
  • Strong assessment, documentation, and organizational skills
  • Compassionate, patient-centered approach to resident care
  • Willingness to work rotating weekends
  • Graduate of an accredited professional nursing program, currently licensed to practice per state.
  • Work experience in the geriatric field
  • Ability to communicate well and in writing.
  • Ability to deal with visitors, staff and residents in a courteous manner.
  • Strong organizational skills.
  • Must be able to work under pressure at times.
  • Must be able to perform good grooming practices, personal hygiene and be punctual and regular in attendance.
  • Must have warm, friendly, congenial personality.
  • Must present a professional appearance and possess an empathetic attitude towards seniors.
  • Ability to assess a resident’s condition and intervene when needed.
  • CPR certified, or obtained within 90 days of hire and yearly thereafter.

Responsibilities

  • Support and oversee day-to-day nursing care for residents in Memory Care
  • Serve as a clinical resource and leader for care staff, providing guidance, education, and support
  • Assist the Wellness Director with assessments, care planning, and ensuring regulatory compliance
  • Monitor resident conditions and respond to changes in health status
  • Administer medications and treatments in accordance with physician orders and state regulations
  • Promote a safe, structured, and person-centered environment for residents with dementia
  • Foster strong communication with families, team members, and interdisciplinary partners
  • Supervise and monitor all memory care nursing staff to obtain highest quality care by performing routine nursing duties.
  • Accountable to Director of Wellness/Executive Director for all memory care nursing department activities and maintaining the corporate philosophy of quality care.
  • Supervise HHAs, CNAs and the facility staff.
  • Make daily rounds of all Memory Care residents.
  • Observes and reports symptoms and conditions of patients to Director of Wellness “DOW” /Delegating R.N./Physician.
  • Administers medications and treatments per signed doctor’s orders in an accurate, safe manner, observes effectiveness and reports to physician if needed.
  • Informs DOW of when supplies and equipment are needed.
  • Orders medication from pharmacy when needed.
  • Supervises the Home Health Aide to ensure that all nursing care is given properly, safely and of the up most quality.
  • Administers emergency first aid.
  • Assures that daily assignments for patient care, incidental duties, performed by nurse and HHA as outlined by DOW are carried out.
  • Re: Nurse/HHA documentation, Ex: V/S, weights, nurse’s notes, medication, kardex, etc.
  • Make daily rounds of all patients and area.
  • Recognize, interpret and report conditions to the DOW/RN/ED as needed.
  • Maintain cleanliness and neatness of nurse’s station, med room and cart.
  • Assist in the orientation of new staff.
  • Complete admission and discharge evaluations as necessary.
  • Check equipment for cleanliness and condition.
  • Provide guidance for nursing assistants.
  • Promote and maintain good interpersonal relationships with residents, visitors and personnel.
  • Perform related work as required per assignment.
  • Understands duties and responsibilities and ensures they are carried out for Memory Care residents.
