About The Position

Chandler Hall, a non-profit, Quaker facility, provides high-quality care for older adults in Newtown, Bucks County and offers a wide spectrum of services for individuals at all stages of the aging process including Community Living, Personal Care, Residential Hospice, Home Hospice, Independent Living, Respite Care, and Skilled Nursing Care.

Requirements

• Licensed Practical Nurse (LPN) qualification
• Rotating weekends, day/night shift

Responsibilities

• Provide clinical oversight, medication management, resident assessments, care coordination, staff supervision, and ongoing monitoring of residents.
• Work collaboratively with caregivers, medication technicians, physicians, families, and other healthcare professionals to promote resident safety, dignity, independence, and quality of life.
• Provide direct nursing care to residents in accordance with applicable laws, regulations, facility policies and individualized care plans.
• Perform medication reconciliation, medication reviews, controlled substance counts, and medication documentation as required.
• Promote a safe environment and identify potential risks related to falls, elopement, medication access, infection control and resident behaviors.
• Respond appropriately to emergencies and initiate appropriate interventions within the scope of practice.
• Communicate effectively with residents, families physicians, pharmacies, home health agencies, hospice providers and other members of the healthcare team.
• Assist in training and monitoring of medication administration.
• Perform resident assessments.
• Supervise and support care staff/ med-techs in accordance with applicable laws, regulations, & facility policies.
• Maintain ongoing compliance with chapter code 2600 regulations.
