The HSD supervises and directs Harmony Square team members, including the Harmony Square Life Enrichment Assistant and PCAs/CNAs, in conjunction with the Healthcare Director. Continuous assessment of resident needs in partnership with the community clinical team, with updates to the Healthcare Director and Executive Director regarding care and changes in resident condition, is crucial, along with initiating interventions as necessary. The HSD participates in Resident/Family Care Conferences and the Family Call Program as directed by the Executive Director. Building relationships with residents and families to encourage participation and promote satisfaction is essential. Maintaining a stimulating environment that promotes safety, security, and overall well-being, in collaboration with the clinical team, is also a core function. The role includes maintaining the Behavior Expression process and ensuring all aspects of dining are met through partnership with the Director of Dining and Healthcare Director. Completing state-required care plans and service plans for Harmony Square residents in conjunction with the Healthcare Director is required. The HSD partners with the Alzheimer’s Association and other local organizations to schedule, organize, and participate in monthly Support Group meetings. Completing necessary training and insurance requirements to drive a company vehicle is also part of the role. Strong leadership skills and the ability to coordinate care across disciplines are necessary. The HSD completes daily and bi-weekly neighborhood rounds, leads and organizes daily huddles, and attends Monthly Programming calls. Promoting a culture of teamwork and team member appreciation, establishing and maintaining positive working relationships with all departments, and effectively communicating with resident families by responding to questions/concerns promptly are also key aspects of the position.
