Requirements

  • A degree in social work, nursing, psychology, gerontology or therapeutic recreation or related field; non-degreed individuals with at least 5 years direct dementia-related resident experience preferred
  • Minimum of two (2) years' experience within a senior living environment specializing in Alzheimer’s and other dementia-related diseases in a “hands-on” supervisory capacity
  • At least 1-year experience managing staff, including coaching, counseling, and progressive disciplinary action
  • Must possess a passion for working with seniors with dementia and improving their quality of life.
  • Thorough understanding of Alzheimer’s and other dementia-related diseases.
  • Excellent communication, organization, and time management skills.
  • Basic typing skills and computer knowledge; familiarity with MS Word, Excel, Publisher, and creative software.

Responsibilities

  • Serve as the dementia care expert for the community, leading all aspects of the Memory Care Program.
  • Ensure the well-being, safety, and comfort of residents.
  • Oversee memory care staff, including hiring, training, scheduling, supervision, coaching, counseling, and performance reviews.
  • Implement Maplewood standards and best practices.
  • Develop and implement specialized service plans tailored to the unique needs of each resident.
  • Collaborate with department heads, family members, program staff, and care staff to ensure the highest quality of care.
  • Manage recruitment for memory care associates and complete monthly schedules.
  • Monitor assigned tasks for all memory care program associates.
  • Update communication logs and lists with new employees.
  • Conduct monthly checks of pendant and wall alarms.
  • Support payroll by reviewing timesheets and entering missed punches/vacation requests.
  • Order necessary resident care supplies.
  • Staff the Program to meet population needs within the approved budget.
  • Ensure Memory Care Program meets all federal and state regulations and company standards.
  • Collaborate effectively with department heads to meet resident and family needs.
  • Relate professionally and positively to residents, physicians, regulators, visitors, and associates.
  • Communicate staffing issues or concerns to the Resident Service Director, Executive Director, and other department heads.
  • Attend weekly Department Head meetings, monthly All Associate meetings, and bi-weekly Memory Care Director meetings.
  • Approve time cards for all memory care associates.
  • Collaborate with RSD on all memory care services plans and participate in their review.
  • Participate in weekly tracking meetings to communicate daily information and observations.
  • Ensure residents are groomed and dressed properly according to personal preferences.
  • Ensure proper outside services are utilized for residents (psych, PT, hospice, etc.).
  • Ensure care is scheduled according to resident's lifestyle, needs, and desires.
  • Ensure all prospects for memory care meet admission criteria and have a diagnosis of dementia.
  • Provide pre-admission screen assessments per MSL standards.
  • Ensure all resident care documentation is completed by care staff prior to end of shift.
  • Coordinate and facilitate family meetings for all memory care residents.
  • Provide stand-up/inter-shift huddles for all memory care neighborhood staff.
  • Provide Person Centered Dementia Care Training to all new staff and ensure all staff are trained on PCDC.
  • Provide monthly dementia-related in-services to all associates.
  • Provide coaching, mentoring, and micro-trainings regularly.
  • Ensure all staff have the minimum training requirements per MSL standards.
  • Assist with touring and providing information about the memory care program to prospective residents' families.
  • Present to outside groups and organizations as needed.
  • Coordinate annual Alzheimer’s Association Walk Team and logistics.
  • Develop and distribute monthly newsletters.
  • Develop and maintain contact with community agencies, organizations, and healthcare professionals.
  • Ensure residents can participate in the meal program independently.
  • Ensure associates provide supervision and assistance in the dining room.
  • Ensure proper nutrition, snacks, and hydration are provided.
  • Ensure dining tables are properly set.
  • Ensure adaptive devices are utilized as recommended.
  • Ensure adequate meals are available for residents who may not eat at normal meal times.
  • Ensure menus are posted or available to residents.
  • Ensure show plates are used for residents to make menu choices.
  • Maintain a resident diet book with photos, diets, consistencies, and allergies.
  • Ensure associates wear approved uniforms during meals.
  • Follow emergency procedures and implement all safety practices.
  • Conduct daily environmental rounds to ensure safety and proper storage of potentially harmful items.
  • Ensure mag locks and safety devices are working to prevent elopement.
  • Ensure all memory care associates use the communication system and mobile devices are in good working order.
  • Ensure elopement kits are fully stocked.
  • Provide weekly behavioral brainstorming meetings to discuss resident behaviors and non-pharmaceutical approaches.
  • Implement behavior logs for residents with a history of aggression or agitation.
  • Ensure associates use behavior logs to report new behavioral occurrences.
  • Ensure an elopement binder is in place for each community.
  • Provide family education on dementia care topics.
  • Provide monthly family support group meetings.
  • Provide frequent communication with families.
  • Maintain a community Memory Care Resource Library.
  • Implement a volunteer program including recruitment, interviewing, training, and recognition.
  • Implement community policy regarding volunteers.
  • Train all volunteers in dementia care.
  • Implement volunteer schedules.
  • Develop and implement intergenerational programs.
  • Develop and implement a volunteer pet therapy program.
