The Memory Care Coordinator is responsible for leading, managing, and overseeing the performance of the memory care team. This role involves ensuring the right care is provided at the right price with the right staff, overseeing various aspects of resident care including clinical functions, life enrichment, culinary services, and living environments. The coordinator is also responsible for training staff on dementia-related topics, hosting support groups and family nights, and providing direct care and communication with residents and their families. A key aspect of this role is adherence to the 1440 care standards and performing resident evaluations to set measurable goals and outcomes. The position requires performing admission and ongoing evaluations, obtaining necessary orders for care plan changes, verifying medication and treatment administration, and reviewing occurrence reports to address resident needs and personnel issues. The Memory Care Coordinator also schedules educational sessions for residents, communicates emergency policies, and works towards continuous improvement of the organization. Flexibility in scheduling, including on-call duties, weekends, and holidays, is necessary.
