About The Position

The Memory Care Coordinator will provide individualized care to residents in Memory Care by providing a therapeutic environment that maintains residents’ highest level of physical, social and psychosocial well-being. This position is part of an on-call rotation.

Requirements

  • Minimum of 2 years’ experience working with geriatric patients, specifically those living with Alzheimer’s or dementia in a memory care or other long-term care environment preferred.
  • Previous programming experience and credentialed as an Activities Coordinator preferred.
  • Current CPR and First Aid certification.
  • Demonstrated written and verbal communication skills.

  • Understand the importance of resident rights in healthcare as well as the community as a whole, and advocate for operations, processes, and decisions that are in residents’ best interests.
  • Coordinate the planning and conducting of group and individual activities for residents that promote emotional and spiritual needs and improve quality of life.
  • Coordinate with family and friends of residents to enhance and promote the dining experience as well as offer a high quality of nutrition.
  • Ensure timely publication of monthly activity calendar and schedule of events that support interests of current residents.
  • Communicate with other staff regarding resident’s activity needs, behaviors, and areas of interest during non-business hours.
  • Conduct Resident Council meetings monthly and receive resident feedback and suggestions on activities, food, care, and other aspects of life. Maintain written records and report suggestions
  • Participate in the Memory Care quality improvement programs and initiatives.
  • Supervise and evaluate the effectiveness of the Halcyon program implementation. Know, understand, and evaluate the effectiveness of the program’s vision and values.
  • Assist in admission process by conducting move-in assessments, interviewing the resident and their family, determining the resident’s daily routine, and providing orientation.
  • Schedule and participate in Care Conferences and make supporting care recommendations based on resident needs.
  • Promote teamwork and continuity of care at all times. Incorporate choice, dignity, privacy, and independence into job performance on a daily basis and encourage other staff to do the same.
  • Train and educate staff on the vision, values, and pillars of the Halcyon program, as well as the fundamentals of diseases causing memory impairment.
  • Assist in scheduling, work assignments, performance evaluation, discipline and counseling in the Memory Care Center.
  • Review and communicate regulatory compliance and new policies or procedures.
  • Perform other duties as assigned.
