About The Position

River Oaks of Minnesota is seeking a compassionate, creative, and dedicated Memory Care Coordinator to lead our Memory Care Program and enhance the lives of residents living with Alzheimer's disease and other forms of dementia. The Memory Care Coordinator is responsible for planning, implementing, and overseeing River Oaks of Minnesota's Memory Care Program. This position creates meaningful daily experiences that enhance the emotional, physical, cognitive, and social well-being of residents living with dementia. Working closely with residents, families, caregivers, nursing staff, and leadership, the Memory Care Coordinator develops individualized programming, provides dementia education and support, and helps create a safe, engaging, and welcoming environment where residents can thrive.

Requirements

High school diploma or GED required.
Minimum of one (1) year of experience working with older adults, individuals living with dementia, mental illness, developmental disabilities, or other vulnerable populations.
Ability to pass all required background studies.
Ability to pass a Minnesota Department of Human Services background study.

Nice To Haves

Associate's or Bachelor's degree in Therapeutic Recreation, Human Services, Psychology, Gerontology, Social Work, Recreation Therapy, Activities, or a related field preferred.
Previous experience coordinating activities or memory care programming.
Certified Dementia Practitioner (CDP) or willingness to obtain certification.
Experience in assisted living, memory care, long-term care, adult day services, or behavioral health.
Knowledge of Alzheimer's disease, related dementias, and person-centered care practices.
Experience planning group activities, special events, or therapeutic programming.
Associate's or Bachelor's degree in Therapeutic Recreation, Recreation Therapy, Gerontology, Human Services, Social Work, Psychology, Activities, or a related field preferred.
Previous experience coordinating activities, life enrichment programming, or memory care services preferred.

Responsibilities

Develop, implement, and coordinate a comprehensive Memory Care Program.
Plan and facilitate daily activities that support residents' cognitive, emotional, physical, social, and spiritual well-being.
Create individualized engagement opportunities based on each resident's interests, abilities, and life experiences.
Adapt programming to meet the changing needs of residents throughout the progression of dementia.
Encourage resident participation while respecting individual preferences and abilities.
Evaluate resident engagement and continuously improve programming based on resident outcomes.
Maintain a monthly calendar of therapeutic and recreational activities.
Build meaningful relationships with residents through consistent interaction and engagement.
Promote dignity, independence, and person-centered care in every resident interaction.
Observe residents for changes in mood, behavior, participation, or overall well-being and communicate concerns to the nursing team and leadership.
Support residents during transitions, new admissions, and changes in routine.
Encourage positive social interaction among residents.
Utilize individualized approaches to reduce anxiety and promote comfort for residents living with dementia.
Develop positive relationships with residents' families.
Encourage family involvement in activities, celebrations, and special events.
Provide education regarding the Memory Care Program and person-centered dementia care.
Participate in family meetings when requested.
Serve as a resource for families regarding available community programs and memory care services.
Assist with educating caregivers on dementia care best practices.
Coach staff on effective communication techniques and meaningful resident engagement.
Promote a culture of compassion, respect, and person-centered care.
Collaborate with Nursing, Life Enrichment, Dietary, Housekeeping, Social Services, Maintenance, and Administration to provide exceptional resident experiences.
Participate in interdisciplinary team meetings and contribute to resident-centered care planning.
Coordinate holiday celebrations, themed events, support groups, outings, educational presentations, and family events.
Build partnerships with volunteers, entertainers, schools, churches, and community organizations.
Coordinate intergenerational programming and special events.
Create a welcoming, engaging, and dementia-friendly environment throughout the community.
Ensure activity areas are organized, safe, and inviting.
Maintain accurate documentation of resident participation and program attendance.
Track resident engagement and evaluate program effectiveness.
Assist in maintaining compliance with Minnesota Assisted Living regulations related to resident activities and quality of life.
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
Support regulatory surveys and inspections as needed.
