About The Position

This position reports to the Assisted living Director and partners closely with Clinical Leader/Case Manager. This role is the anchor to the neighborhood, ensuring adjustments in staffing within individual staffing plan parameters, supervising and directing service plan creation and implementation through leadership of resident care aides and medication assistants, completing day to day procedures, following up with PCPs and supporting RN Clinical Leader/case manager in maintaining documentation and communication as well as ensuring resident safety and that all care needs are met according to the Department of Health regulations and Glenmere policy. Works collaboratively as a part of the Glenmere Leadership team to ensure an elevated training, onboarding, and employment experience for RCA and Med tech team members and residents.

Requirements

  • LPN with 1-2 years’ experience in supervision
  • 1-2 years’ experience as a resident care giver in a health-related field or an assisted living facility.
  • Experience assisting with medications is required.
  • High school diploma or GED required.
  • Ability to proficiently read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully and effectively with internal and external customers including residents, family members, visitors, staff, government agencies, vendors and business associates, and the general public.
  • Must be knowledgeable of policies and procedures, as well as DOH regulations pertaining to assisted living levels of care (ALR, EALR, and SNALR).
  • Must have reliable transportation.

Nice To Haves

  • Previous supervisory experience is preferred.
  • Valid state of residence driver’s license preferred

Responsibilities

  • Oversees and directs RCA and med tech team, including interviewing, hiring, and welcoming new direct care employees to their team, ensuring a welcoming and complete training for new employees.
  • Completion of 90-day and annual staff performance appraisals, to include employee performance plans when required.
  • Assisting with planning, developing, organizing implementing, evaluating and supervising resident care services, including assistance with medication in accordance with current rules, DOH regulations and guidelines that govern the community.
  • As a part of the leadership team of Glenmere, meets with administration, medical and Resident care staff, as well as other related departments in planning resident care.
  • Maintains service records, resident files, and progress notes as required with support of the Clinical Leader Case manager and Assisted Living Director.
  • Makes written and oral reports or recommendations to the Assisted living director and Clinical leader/Case Manager, as necessary or required, concerning care services.
  • Maintains a relationship with families and residents and supports the direct care team in doing so.
  • Notes and reports errors in the medications supervision program
  • Reports and investigates any complaints or grievances made by residents in partnership with the AL director and Clinical Nurse leader/case manager maintaining confidentiality and integrity.
  • Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.
  • Provides guidance and training for new and/or less experienced staff.
  • Completes appropriate reports to the Clinical Nurse Leader/Case Manager as required or as may be necessary, including departmental performance evaluations.
  • Assists in identifying and correcting problem areas and/or the improvement of services.
  • Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to the Administrator
  • Attends and participates in workshops, seminars, and in-services as required to keep abreast of current changes in the health care field.
  • Ensures that staff, residents, visitors, etc., follow established safety policies and procedures at all times.
  • Ensures that staff follows established infection control procedures when performing duties and adequate supplies are available.
  • Ensures that all staff are knowledgeable and follow residents' rights and HIPAA guidelines of Resident’s health information.
  • Ensures correctly certified/ trained or licensed team members on duty provide specific assists residents with prescribed injectable medications, dressing changes, and beyond Assisted Living standard assistance permitted by the Department of Health under the licensure guidelines for enhanced and special needs certifications.
  • Supports the overall coverage plan including direct care and med pass assistance if needed and serves as a part of the on call and coverage team
