Provide information and assistance to members, caregivers, providers and other stakeholders in support of the HCBS Unit. This role involves handling incoming emergent communication requests, documenting requests accurately, communicating with members compassionately, and facilitating services based on care plans. The specialist will also manage census data, track facility-based members, distribute satisfaction surveys, and assist with service referrals. Additionally, they will monitor UM alerts for specific programs and complete non-licensed visits as needed.
