About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on providing care for children with special healthcare needs. They offer 24/7 virtual and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to improve access to quality care. The Medication Support Navigator is part of the clinical pharmacy team and aims to increase safe days at home for children with special health and behavioral care needs by improving medication-related outcomes and supporting medication access. This role is primarily remote with quarterly travel for training. The schedule rotates to accommodate later appointment times, with potential for shifts in PST. Bilingual Spanish is highly preferred.

Requirements

  • Minimum of 3 years of experience as a pharmacy technician.
  • High School Diploma or equivalent.
  • Current accredited pharmacy technician certification (such as PTCB).
  • Maintenance of certification expected.
  • Registration with applicable state board(s) of pharmacy as required by law.
  • Maintenance of all technician registration for all assigned states expected.
  • Proficiency with electronic health record systems.
  • Bilingual English/Spanish highly preferred.

Nice To Haves

  • Pediatric experience preferred.
  • Ambulatory clinic or specialty pharmacy experience preferred.

Responsibilities

  • Manage, triage, and assign clinical pharmacy referrals to appropriate clinical pharmacy team members.
  • Support internal and external team members in the clinical pharmacy referral process based on patient needs.
  • Schedule patient/caregiver appointments for pharmacist-driven clinical programs.
  • Prepare charts in advance of Clinical Pharmacist appointments.
  • Communicate with pharmacies, insurance companies, and internal team members to resolve medication access barriers with pharmacist support.
  • Complete medication histories.
  • Monitor incoming medication renewal requests and manage per internal protocols, seeking assistance when outside renewal protocols.
  • Monitor incoming prior authorization requests and prepare documentation for review by clinical pharmacist.
  • Demonstrate the ability to set priorities, work independently (with pharmacist oversight and review of final product) and cooperatively with others within and outside the department.
  • Perform other duties as assigned.
