About The Position
The pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
Requirements
- Nationally certified as a pharmacy technician.
- 6 months pharmacy experience.
- Registration with Indiana Board of Pharmacy within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire.
Responsibilities
- Obtaining medication and allergy histories for patients admitted to the hospital.
- Interviewing patients, patient families, calling pharmacies, long term care facilities and/or other providers to gather medication information.
- Communicating obtained medication information to pharmacists and healthcare providers to determine treatment plans and medication orders during hospitalization and post discharge.
