About The Position
MercyOne is seeking a Medication History Pharm Tech to provide technical support in obtaining, verifying, and recording initial medication histories from patients and/or their designated family/caregivers. This role involves conducting interviews to gather information on current medication regimens, including prescriptions, over-the-counter medications, vitamins, and herbal supplements. The position also requires performing other functions of a pharmacy technician as allowed by Iowa law. MercyOne is committed to improving community health and providing personalized, convenient care. Requirements
- High School Diploma or GED
- National Pharmacy Technician Certification and Registered with the Iowa Board of Pharmacy at the time of hire OR Registered as a Pharmacist-Intern with the State of Iowa at time of hire
- 2,000 hours of pharmacy technician experience
- Successfully complete Medication History Pharmacy Technician Competency/Skill Set.
Responsibilities
- Provide technical support in obtaining, verifying for accuracy, and recording initial medication histories from patients and/or their designated family/caregivers upon presentation to Mercy patient care areas.
- Conduct patient and/or family interviews focused on obtaining current medication regimens including prescription and over-the-counter (non-prescription) medications, as well as vitamin and herbal supplements.
- Perform other functions of a pharmacy technician registered in the State of Iowa and within the scope of Iowa law as needed.
- Interview patients and/or their designated family/caregivers as soon as possible after patient presents for care to MercyOne clinical areas (includes the Emergency Department, as well as inpatient and outpatient areas).
- Obtain and record in the electronic medical record a current list of medications to include: prescription medications, over-the-counter (non-prescription) medications, vitamin and nutritional supplements, as well as the source(s) for this information.
- Record medication details including: name/location of patient’s ambulatory pharmacy, medication/supplement name, dosage form, strength, dose, day/time of most recent dose, and source(s) of the information being recorded.
- Attempt to clarify as many uncertainties and compile a Best Possible Medication History utilizing patient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facility records and information from previous hospital encounters.
- Obtain and record in the electronic medical record an allergy history for each patient, clarifying and recording the nature of the allergic reaction when necessary.
- Consult with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication and allergy history.
