Under the general direction of the hospital pharmacy, Medication Collection Pharmacy Technicians independently perform tasks related to collecting, reconciling, and entering the home medication history into the patient's electronic medical record. This role involves interviewing patients and family members, contacting local pharmacies, and utilizing other resources to compile a comprehensive medication history. The technician will then enter this information into the patient's electronic health record for review by the admitting provider and use by physicians and other healthcare professionals to determine treatment plans and medication orders during and after hospital discharge. The position requires interaction with nurses, physicians, pharmacists, and other medical professionals to provide medication history and related patient information, ultimately improving patient safety and quality by ensuring physicians have a complete home medication list. The technician may also perform various functions that impact the medication reconciliation process and patient care, and will assist the pharmacist in their professional responsibilities, including the distribution of medications and tasks associated with regulatory compliance.
