About The Position

Lamoille Health Partners is seeking a dedicated Medication-Assisted Treatment (MAT) Counselor to join our integrated Community Health Team in Morrisville, Vermont. In this role, you will be a vital lifeline for individuals navigating substance use disorders, helping them break down barriers, rebuild their lives, and find long-term healing within a supportive primary care setting.

Requirements

Hold an active, unencumbered professional license in the State of Vermont (e.g., LCMHC, LICSW, or LDAC/CADC equivalent) within your professional scope of practice.
Have a strong foundation in substance use disorder (SUD) treatment, harm reduction principles, and trauma-informed care.
Excel at building trusting, non-judgmental relationships and utilize motivational interviewing with ease.
Thrive in a fast-paced, multidisciplinary team environment and know how to advocate effectively for your patients.

Responsibilities

Provide individual counseling, crisis intervention, and relapse prevention strategies tailored to patients undergoing medication-assisted treatment.
Utilize motivational interviewing and comprehensive psychosocial evaluations to identify barriers to care, social determinants of health, and individual recovery goals.
Partner seamlessly with our medical and behavioral health teams to manage patient registries, track progress, and ensure smooth transitions across levels of care.
Connect patients and their families with essential community services, housing resources, peer support networks, and financial assistance programs.
Participate in team huddles, quality improvement initiatives, and data tracking to ensure our MAT program continues to meet the highest standards of care.
