Medicare Wellness Registered Nurse
About The Position
The Registered Nurse (RN) works in multiple physician offices to provide professional nursing care for patients, including assessment, treatment, care planning, medical care evaluation, and education, following established standards and practices. The RN float assists providers and staff in coordinating patient care and serves as a liaison between patients, staff, and providers. This full-time Medicare Wellness RN processes Medicare Wellness Visits at Lafayette Family and Primary Practice locations. The position may report to multiple locations in the Lafayette area and follows a Monday-through-Friday schedule.
Requirements
- Associate's degree required.
- Valid RN license to practice in Indiana or as listed in the Nurse Licensure Compact (NLC) upon hire.
- Basic Life Support (BLS) through the American Heart Association.
Nice To Haves
- Bachelor's degree preferred.
- Two years of nursing or patient-care experience preferred.
Responsibilities
- Process Medicare Wellness Visits.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care.
- Maintain accurate, detailed reports and records.
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Prepare patients for and assist with examinations or treatments.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Assist providers and staff in coordinating patient care and act as a liaison between patients, staff, and providers.
