About The Position

Activa Home Health, a proud member of the AccordCare family of companies, is seeking an experienced Medicare Home Health Scheduler to support the clinical side of our home health operations. This role is responsible for coordinating and scheduling patient visits for our clinical team, ensuring patients receive timely and consistent care in their homes. The Scheduler will work closely with Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Physical Therapists (PTs), Occupational Therapists (OTs), Speech Therapists (STs), Physical Therapist Assistants (PTAs), and Speech Therapy Assistants (STAs). The ideal candidate is highly organized, able to manage multiple clinician schedules, and thrives in a fast-paced healthcare environment.

Requirements

  • Minimum 1 year of Home Health Scheduling experience required
  • Experience with Medicare Home Health scheduling strongly preferred
  • Experience scheduling RNs, LPNs, and therapy disciplines (PT, OT, ST) preferred
  • Strong organizational and multitasking abilities
  • Excellent communication and customer service skills
  • Ability to work in a fast-paced clinical environment
  • Experience with EMR systems in Home Health preferred
  • NEGATIVE TB Test / NEGATIVE QuantiFERON Blood Test
  • This role requires a screening through the Care Provider Background Screening Clearinghouse.

Responsibilities

  • Schedule and coordinate patient visits for RNs, LPNs, PTs, OTs, STs, PTAs, and STAs.
  • Manage clinician schedules to ensure efficient patient coverage and visit completion.
  • Schedule visits according to physician orders and Medicare home health guidelines.
  • Adjust schedules based on clinician availability, patient needs, and urgent visit requests.
  • Monitor daily schedules to ensure visits are completed and properly documented.
  • Communicate daily with clinical managers, intake, and case managers regarding new admissions and schedule changes.
  • Serve as a key point of contact for clinicians regarding schedules and patient visits.
  • Coordinate scheduling for new patient admissions, resumption of care visits, and follow-up visits.
  • Assist with resolving scheduling conflicts while maintaining continuity of care.
  • Maintain and update clinician schedules in the EMR system.
  • Ensure visits align with authorized services and physician orders.
  • Track scheduled visits and assist in maintaining accurate scheduling records.
  • Provide professional communication with patients, clinicians, and internal teams.
  • Confirm patient availability and coordinate appointment times when necessary.
  • Maintain confidentiality of all patient information in accordance with HIPAA and company policies.
