Under the direction & supervision of the Director of Population Health, the Patient Navigator is responsible for conducting patient education, assisting patients to complete health history forms, self-assessments & risk assessments & entering results into the electronic health record (EHR), representing Indian Health Center (IHC) at health fairs & community events, assisting with clinic close care gaps for the patients. He/she will participate in organizational improvement activities as needed around chronic disease management. This position includes working and collaborating with vendors and performing other activities as requested. In addition, this staff will perform Medical Transcription duties by interpreting and transcribing external clinical notes regarding patient assessment, workup, therapeutic procedures, clinical course, diagnosis, prognosis, etc. into the patient’s Electronic Health Records in order to document patient care and facilitate delivery of health care services. He/she ensures accuracy of electronic clinical notes and keeps confidentiality in compliance with HIPAA guidelines.
