About The Position

Concordia Home Health of Bethlen is hiring a part-time Medical Social Worker to serve patients in Ligonier Valley and surrounding areas. This role is part of an interdisciplinary team, providing essential medical social services. Requirements

Master’s degree from a school of social work approved by the Council on Social Work Education.
One year of social work experience in a healthcare setting.
Reliable transportation for home visits, with a valid driver’s license and insurance as per personnel policies.

Responsibilities

Provide rehabilitative and supportive casework to help patients achieve optimal social and health adjustment.
Assist patients and families in understanding and following medical recommendations.
Develop care plans collaboratively with physicians, patients, families, and team members, adjusting as needed based on patient conditions.
Facilitate access to community resources, acting as an intermediary when necessary.
Address personal and environmental challenges that hinder patients from maximizing their medical care benefits, including counseling and assisting with admissions to care facilities.
Supervise the treatment program of the social worker assistant, reviewing and co-signing clinical visit notes as appropriate.
