VNA is looking for a Medical Social Worker. The Medical Social Worker evaluates the social and emotional factors affecting a patient’s illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients. Collaborate with the healthcare team to develop individualized treatment plans and actively participates in patient care conferences.

Requirements

• Master of Social Work degree
• At least one year of social work experience in healthcare
• Possess a current CPR certification
• Current California driver's license
• Auto insurance
• Reliable vehicle

Responsibilities

• Evaluates the social and emotional factors affecting a patient’s illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients.
• Collaborate with the healthcare team to develop individualized treatment plans.
• Actively participates in patient care conferences.
