About The Position

Aspire Home Healthcare of Northern California is seeking a part-time Medical Social Worker (MSW) for the Yuba City area. This is a paid per visit position. Aspire Home Healthcare was established 30 years ago with the purpose of putting patients first, providing around-the-clock care and support. The MSW will provide evaluation and treatment to patients experiencing social and personal adjustment problems that interfere with independent living or are a reaction to their medical diagnosis. Responsibilities include developing treatment plans and providing individual, family, or group therapy.

Requirements

  • Master’s degree in medical social work or social work license in CA
  • 2 years of social work experience in a medical setting, preferably in home health
  • Current CPR Certification
  • Valid driver’s license and car insurance
  • Ability to attend 2-3 day orientation at our Yuba City office

Responsibilities

  • Provides psychosocial evaluation, diagnosis, treatment, and referral for patients and/or families. Evaluation shall include patient and/or family goals.
  • Interprets results of evaluation and recommends treatment as indicated to the patient/family referral source.
  • Provides services that include but are not restricted to community referral, individual therapy, family therapy, group therapy, consultation, or education.
  • Aids the health care team in identifying, understanding and integrating the significance of psychosocial factors in relation to the patient’s illness, treatment, and recovery.
  • Plans and leads individual and family therapy, patient and family group and leads family conferences.
  • Provides a written report for patient medical record (evaluation, progress notes, and discharge summary) with copies to referral source, patient, and family when indicated.
  • Maintains accurate, up-to-date records in the client’ chart.
  • Consults with physicians, families, and referral source regarding client’s progress and therapy program.
  • Maintains all departmental records in accordance with company’s policy, state and federal regulations.
  • Maintains confidentiality of client’s situation, records and services provided according to HIPPA regulations.
  • Discusses with the Director of Clinical Services problems concerning the patients and how these may best be resolved, changes in patient needs and how these can be best met, and need for involvement of other members of health team.
  • Coordinates total patient care.
