This role involves completing psychosocial assessments for patients' post-hospital care, assisting them in coping with stress related to hospitalization and illness, and collaborating with the Clinical Care Management team to determine appropriate care settings. The Medical Social Worker will serve as a resource to hospital staff and physicians regarding the emotional and social aspects of a patient's illness and its impact on their support system. They will also provide information and education on community resources and post-hospital care options, make referrals to community agencies, report suspected cases of abuse/neglect/exploitation, and act as a liaison with the Department of Social Services. Additionally, this position will respond to requests for consultative services after normal working hours and take on-call duty as scheduled.
