About The Position

At Aegis Healthcare, we are expanding our Aegis Home Health and Hospice division, which provides home health and hospice care for patients in their homes and senior living communities and are seeking a dynamic and experienced individual for the role of Medical Social Worker (MSW) who shares our desire to make healthcare better for everyone. Requirements

  • Master's Degree in Social Work from an accredited University.
  • Valid licensure in AZ
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work independently, be self-directed, and flexible.
  • High level of confidentiality due to the sensitivity of materials and information.
  • Effective verbal & written communication & time management skills
  • Valid Driver's License (clean record) & reliable transportation.
  • Proof of current auto insurance & registration
  • Valid AZ Fingerprint Clearance Card (we can help)
  • Current CPR Certification (we can help) & Negative TB Test (we can test)

Responsibilities

  • Provide short-term individual counseling, crisis intervention, and assist with funeral planning issues and the transfer of responsibilities regarding fiscal, legal, and health care decisions.
  • Document all findings, plans, interventions, and client progress in the clinical record.
  • Assist patients/families in assessing financial resources.
  • Address patient/family questions and issues.
  • Identify family dynamics and communication patterns.
  • Evaluate the patient/family response to intervention(s) when referred to community agencies and satisfaction with the service(s) provided.
