About The Position
The Medical Social Worker provides psychosocial support, assessment, and care coordination to patients and families receiving services in the home. The role addresses emotional, social, financial, and environmental factors that affect health and recovery and supports safe, effective care transitions. Working closely with the interdisciplinary team, the Medical Social Worker connects patients with community resources, provides counseling, and promotes optimal outcomes in the home setting.
Requirements
- Master's and/or doctorate degree from a school of social work accredited by the Council on Social Work Education.
- Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
- Current CPR certification.
- At least 1 year of social work experience in a healthcare setting.
- Meet state and federal requirements for the position.
Responsibilities
- Assist patients and families with health-related financial, social, and emotional concerns and support effective coping skills.
- Provide social work services in accordance with the plan of care.
- Communicate changes in a patient's clinical status promptly to other healthcare team members and care center office staff.
- Educate patients, family members, and other team members about medical social work interventions and the treatment plan.
- Perform care planning, transfer, and discharge functions.
- Identify risks or threats to a patient's health in their environment, promptly implement appropriate interventions, and communicate identified risks to the physician and other healthcare team members to coordinate care.
- Perform other duties as assigned.
