About The Position
Accelerated Home Health Care is hiring a Medical Social Worker (MSW) to provide in-home psychosocial support to patients and families. This role involves conducting assessments, providing counseling, connecting individuals with community resources, documenting interventions, and ensuring adherence to care plans and regulations.
Requirements
- MSW degree and state licensure.
- Completion of agency onboarding/credentialing.
- Up-to-date TB test.
- Reliable transportation.

- Ability to work independently in a home-based setting.
