About The Position

Western North Carolina Community Health Services (WNCCHS), a Federally Qualified Health Center, is hiring a full-time Medical Social Worker/Medical Care Coordinator. The position coordinates the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to identify options and services that meet individuals’ and families’ comprehensive health needs. The work uses communication and available resources to promote patient safety, quality of care, and cost-effective outcomes. The role is based at Minnie Jones Health Center, 257 Biltmore Ave, Asheville, NC 28801.

Requirements

Bachelor’s degree in human services, social work, or psychology.
At least 2 years of previous experience in care or case management.
Motivational interviewing skills.
Knowledge of stages-of-change interventions.
Demonstrated ability to facilitate the execution of care plans.
Expert knowledge of medical office operations.
Familiarity with the role of FQHCs in the community.

Nice To Haves

Master’s degree.
