Medical Social Worker - Atrium Health Levine Children's Hospital Child Protection Team - FT Days
About The Position
This is a full-time Medical Social Worker position with Atrium Health Levine Children's Hospital Child Protection Team. The role involves completing psychosocial assessments, assisting patients in coping with stress related to illness and hospitalization, and collaborating with the Clinical Care Management team to determine appropriate care settings. The Medical Social Worker will also serve as a resource to hospital staff and physicians on psychosocial aspects of patient care, provide information and education on community resources, make referrals to agencies, and report suspected cases of abuse, neglect, or exploitation. Additionally, the role involves serving as a liaison between the hospital, patients, and the Department of Social Services, and responding to consultative requests after normal working hours.
Requirements
- Masters in Social Work required.
- Adherence to National Association of Social Workers Code of Ethics.
- Expertise with Data Management Tools.
Nice To Haves
- LCSW preferred.
- Applicable state Certification is preferred.
- At least 1 year professional experience in hospital or health related setting preferred.
- Specialized knowledge of child abuse, neglect, trauma, and family systems.
- Experience conducting child maltreatment risk assessments.
- Strong crisis intervention and safety planning skills.
- Proficiency in psychosocial assessment and case management.
- Ability to coordinate community resources and support services.
- Excellent documentation and report-writing skills.
- Knowledge of child welfare, legal, and healthcare systems.
- Experience facilitating multidisciplinary meetings and educational sessions.
- Strong data entry, organization, and care coordination abilities.
Responsibilities
- Completes psychosocial assessments for designated patients and their support systems for post-hospital care.
- Assists patients in coping with stress related to hospitalization, disability, and chronic/terminal illness.
- Works with the Clinical Care Management team to assess and evaluate the patient for the appropriate level of care or environment setting to meet care needs across the continuum.
- Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness and its effects on their social support system.
- Provides information and education to patients on community resources and options for post-hospital care appropriate to the age of the patients served, in collaboration with the Clinical Care Management team.
- Makes referrals to community agencies as needed.
- Reports suspected cases of child & adult abuse/neglect/exploitation.
- Serves as liaison between hospital, patients and Department of Social Services during evaluation/investigation.
- Responds to requests for consultative services to patients after normal working hours.
