The medical social worker is responsible for the implementation of standards of care for medical social work services. This role involves assessing the psychosocial status of patients, planning interventions, providing counseling and education to patients, caregivers, and families, and collaborating with physicians and other healthcare practitioners. The social worker will also assist in understanding social and emotional factors related to health problems, prepare clinical notes, provide information and referral services, and act as a liaison between patients/families and community agencies. Additionally, the position requires participation in the total plan of care, case conferences, discharge planning, and quality improvement programs. Supervision of Social Worker Assistants may also be required.
