Overview The Medical Social Worker provides psychosocial support to patients and families, addressing economic, social, and emotional needs within a home health agency setting. This role supports safe and effective care transitions, advocates for patient rights, and contributes to interdisciplinary care planning.
Requirements
- Master’s Degree in Social Work from an accredited institution
- Minimum one year of experience in a healthcare setting (home health experience preferred)
- Strong communication, documentation, and problem-solving skills
- Ability to work collaboratively within an interdisciplinary team
- Commitment to patient-centered, safe, and compassionate care
Responsibilities
- Assess patients and families to identify psychosocial needs related to illness and end-of-life care.
- Evaluate mental health status, insurance coverage, and financial assistance options.
- Develop comprehensive discharge plans based on interdisciplinary input and case management principles.
- Communicate effectively with physicians and the care team to coordinate services.
- Participate in regular team meetings and provide on-call social work coverage as needed.
- Facilitate discharge planning from hospitals to home or nursing facilities.
- Provide counseling and crisis intervention, including guidance on Advanced Directives and bereavement support.
- Maintain timely and accurate clinical records.
- Ensure adherence to agency policies, state and federal regulations, and accreditation standards.
- Contribute to quality improvement initiatives.
- Engage in community education to support program growth and strengthen service reach.
- Protect patient confidentiality and comply with HIPAA.
- Promote patient rights and report concerns promptly.
- Follow infection control and emergency response procedures to maintain a safe care environment.
- Collaborate with patients, families, and colleagues.
- Support staff orientation and ongoing professional development.
- Maintain clinical competency and serve as a positive role model for teamwork.
- Complete required documentation accurately and on time.
- Coordinate with community resources and contracted providers.
- Manage time effectively to meet productivity standards while delivering high-quality care.
