Overview The Medical Social Worker provides psychosocial support to patients and families, addressing economic, social, and emotional needs within a home health agency setting. This role supports safe and effective care transitions, advocates for patient rights, and contributes to interdisciplinary care planning.

Requirements

  • Master’s Degree in Social Work from an accredited institution
  • Minimum one year of experience in a healthcare setting (home health experience preferred)
  • Strong communication, documentation, and problem-solving skills
  • Ability to work collaboratively within an interdisciplinary team
  • Commitment to patient-centered, safe, and compassionate care

Responsibilities

  • Assess patients and families to identify psychosocial needs related to illness and end-of-life care.
  • Evaluate mental health status, insurance coverage, and financial assistance options.
  • Develop comprehensive discharge plans based on interdisciplinary input and case management principles.
  • Communicate effectively with physicians and the care team to coordinate services.
  • Participate in regular team meetings and provide on-call social work coverage as needed.
  • Facilitate discharge planning from hospitals to home or nursing facilities.
  • Provide counseling and crisis intervention, including guidance on Advanced Directives and bereavement support.
  • Maintain timely and accurate clinical records.
  • Ensure adherence to agency policies, state and federal regulations, and accreditation standards.
  • Contribute to quality improvement initiatives.
  • Engage in community education to support program growth and strengthen service reach.
  • Protect patient confidentiality and comply with HIPAA.
  • Promote patient rights and report concerns promptly.
  • Follow infection control and emergency response procedures to maintain a safe care environment.
  • Collaborate with patients, families, and colleagues.
  • Support staff orientation and ongoing professional development.
  • Maintain clinical competency and serve as a positive role model for teamwork.
  • Complete required documentation accurately and on time.
  • Coordinate with community resources and contracted providers.
  • Manage time effectively to meet productivity standards while delivering high-quality care.
