About The Position

This PRN/Part Time Medical Social Worker (MSW) position in Home Health focuses on assisting patients, their families, physicians, and the healthcare team in understanding and addressing personal, emotional, environmental, and social factors that impact a patient's health and their ability to benefit from medical services and the plan of care. The role involves contributing to comprehensive treatment plans, educating the team on available community resources, and maintaining detailed clinical records. The social worker will evaluate the effectiveness of interventions, participate in agency activities, and supervise social work assistants. Continuous self-development to improve practice is also a key aspect of the role.

Requirements

  • A master's degree (MSW) from a school of social work accredited by the Council on Social Work Education, or the individual must be licensed in the State of Texas.
  • One year of experience, working as a social worker.
  • Current driver's license with current automobile insurance.
  • Assesses the client’s potential to cope with his/her social and health problems.
  • Acts as consultant to the members of the health team, assists them in understanding the social, emotional and environmental factors related to the client's health problems.
  • Helps clients to utilize the resources of their families and the community.
  • Provides rehabilitative and supportive casework.
  • Assists clients and their families in coping with personal and environmental difficulties which might predispose them toward illness or interfere with obtaining maximum benefits from medical care.
  • The ability to communicate well, both verbally and in writing is required.
  • The ability to access clients' homes which may not be routinely wheelchair accessible is required.
  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • Physical activities will include, walking, sitting, stooping, standing and minimal to maximum lifting of clients and the turning of clients.

Responsibilities

  • Assists the patients, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors and difficulties related to the patient's health problems; which interfere with maximizing the benefit of medical services and the plan of care.
  • Contributes as a health care team member to the development of comprehensive, integrated treatment plans for patients.
  • Instructs health care team members on community resources available to assist patients.
  • Observes records and reports changes in the client's emotional and social factors that affect the client's illness and his/her need for care and his/her response to treatment.
  • Maintains and submits written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
  • Evaluates the clients and family's response to, and effectiveness of, the medical social work intervention.
  • Participates in Agency activities and committees when appropriate to include staff development activities and in-service education.
  • Supervises the social work assistant once monthly.
  • Assumes responsibility for self-development by continually striving to improve his/her Medical Social Worker‘ practice though formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and reading.
