Medical Social Worker
About The Position

Requirements
- Current Medical Social Worker license in the state of employment (limited licenses not accepted)
- Minimum 1 year of MSW experience in an acute care setting (home care experience preferred)
- Master’s degree in Social Work
- Valid driver’s license and auto insurance
- Ability to meet physical job demands

Responsibilities
- Connect patients with community resources
- Ensure patients’ medical, emotional, and safety needs are met
- Deliver skilled services as ordered by the attending physician
- Educate patients and family members
- Promote patient safety and well-being
- Plan of Care: Participate in developing patient care plans
- Conduct patient assessments and identify barriers
- Observe, document, and report patient changes to appropriate staff
- Patient Outcomes: Connect patients and families to community resources
- Assist with financial aid applications and long-term planning
- Family Educator / Advocate: Counsel patients and families on coping, life changes, and grief
- Policies: Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
- Rules & Regulations: Follow all agency policies, procedures, and regulations
- Communicate schedule or availability changes to supervisors
- Operations: Attend required trainings and agency meetings
