About The Position

As a Medical Social Worker (MSW), the work you do every day makes a meaningful difference in the lives of our patients. Requirements

  • Current Medical Social Worker license in the state of employment (limited licenses not accepted)
  • Minimum 1 year of MSW experience in an acute care setting (home care experience preferred)
  • Master’s degree in Social Work
  • Valid driver’s license and auto insurance
  • Ability to meet physical job demands

Responsibilities

  • Connect patients with community resources
  • Ensure patients’ medical, emotional, and safety needs are met
  • Deliver skilled services as ordered by the attending physician
  • Educate patients and family members
  • Promote patient safety and well-being
  • Plan of Care: Participate in developing patient care plans
  • Conduct patient assessments and identify barriers
  • Observe, document, and report patient changes to appropriate staff
  • Patient Outcomes: Connect patients and families to community resources
  • Assist with financial aid applications and long-term planning
  • Family Educator / Advocate: Counsel patients and families on coping, life changes, and grief
  • Policies: Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
  • Rules & Regulations: Follow all agency policies, procedures, and regulations
  • Communicate schedule or availability changes to supervisors
  • Operations: Attend required trainings and agency meetings
