About The Position
We are seeking a compassionate and skilled Medical Social Worker (MSW) to join our team for per diem home health visits. In this role, you will complete psychosocial assessments, help patients access community resources, and support patients and families through healthcare transitions. You will also address social, emotional, and financial barriers, collaborate with physicians, nurses, and therapists, and maintain documentation. This position offers a flexible schedule and the opportunity to make a meaningful impact in the lives of patients in Martin County.
Requirements
- Master's Degree in Social Work
- Knowledge of community resources
- Strong communication and advocacy skills
