Assists patients, their significant others, physicians, and healthcare team staff in understanding personal, emotional, environmental, and social factors that affect a patient's health problems and interfere with maximizing the benefit of medical services and the plan of care. Contributes to the development of comprehensive, integrated treatment plans for patients as a member of the healthcare team. Instructs healthcare team members on available community resources to assist patients. Observes, records, and reports changes in the client's emotional and social factors that affect their illness, need for care, and response to treatment. Maintains and submits written clinical records, including initial evaluations, care plans, and daily notes, as required by the Agency. Evaluates the client's and family's response to and effectiveness of medical social work intervention. Participates in Agency activities and committees, including staff development and in-service education, as appropriate. Supervises the social work assistant once monthly. Assumes responsibility for self-development by continually improving Medical Social Worker practice through formal education, workshops, conferences, professional meetings, and individual research.
