Viva Life Home Health & Home Care is seeking a compassionate and dedicated PRN Medical Social Worker (MSW) to join our growing home health team. As a Medical Social Worker, you will play a vital role in supporting patients and families as they navigate healthcare challenges, access community resources, and develop plans that promote safety and well-being. Requirements
- Master's Degree in Social Work (MSW) from an accredited school of social work.
- Current Texas licensure as required by state regulations.
- Minimum of one (1) year of social work experience in a healthcare setting preferred.
- Home health experience preferred but not required.
- Knowledge of community resources and healthcare systems within the San Antonio area.
- Strong communication, assessment, and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation and current driver's license.

Responsibilities
- Assess patients' psychosocial status and identify barriers to care.
- Develop and implement individualized plans to address social, emotional, and environmental needs.
- Assist patients and families with accessing community resources and support services.
- Provide counseling and emotional support related to illness, disability, adjustment, grief, and caregiving challenges.
- Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary team.
- Participate in care planning and case conferences.
- Document assessments, interventions, and patient progress in accordance with agency standards and regulatory requirements.
- Educate patients and families regarding available resources, advance directives, and long-term planning options.
- Promote patient rights, dignity, and self-determination.
