About The Position
BAYADA Home Health Care is seeking a Medical Social Worker (MSW) to serve home health and hospice clients and their families in Prescott, Arizona. The role is an integral part of a multidisciplinary health care team that provides skilled nursing and rehabilitative care, helping clients remain at home. The social worker provides clinical evaluations, develops appropriate interventions, connects clients and families with community resources, advocates for them, and contributes to the overall plan of care. The position is per diem and offers flexible scheduling and weekly pay. Paid training and around-the-clock clinical support are provided.
Requirements
- Master’s degree in social work.
- At least two years of community-based experience.
- Accreditation by the Council on Social Work Education, demonstrated by a school transcript or degree.
- Solid computer skills.
- Ability to travel to assigned cases.

- Prior experience with electronic medical records (EMR).
Responsibilities
- Help the care team understand social and emotional factors related to clients’ health problems.
- Conduct clinical social evaluations and plan appropriate interventions based on evaluation findings.
- Identify community resources to address clients’ and families’ practical and environmental needs.
- Advocate for clients and families as they navigate community systems.
- Participate with the multidisciplinary team in developing the total plan of care.
- Develop and maintain contacts with public and private agencies as resources for patients.
- Educate and instruct patients, family members, or other representatives about community resources to promote health, safety, well-being, and independent living.
- Document observations, interventions, and evaluations of patient care and services using a touchpad tablet.
- Travel to assigned cases.
