This is a part-time Medical Social Worker position within the Neonatal Developmental Follow Up Clinic. The role involves conducting psychosocial evaluations, providing counseling and support to patients and their social support systems, and assisting them in coping with the stresses of hospitalization, disability, and chronic/terminal illness. The Medical Social Worker will collaborate with a multidisciplinary team to develop and implement care plans that address identified psychosocial needs, serve as a resource to hospital staff regarding psychosocial aspects of patient care, and intervene in crisis situations. Responsibilities also include completing psychosocial assessments for post-hospital care, working with the Clinical Care Management team to determine appropriate care settings, and participating in multidisciplinary care planning meetings. The role requires maintaining knowledge of community resources, providing information and education on these resources and post-hospital care options, and making referrals as needed. Additionally, the Medical Social Worker will report suspected cases of abuse/neglect/exploitation and serve as a liaison with the Department of Social Services. The position also involves identifying and addressing barriers to healthcare transitions and contributing to system improvements. Accurate case record maintenance and documentation in the medical record are essential.
